Diabetes Management in School
Healthcare Provider orders and supplies for each school year must be brought in before the first day of school.
Diabetes orders must include:
- Action Plan for hyperglycemia and hypoglycemia
- Meal Plan
- Insulin Regimen
- Requirements and Restrictions
- Healthcare Provider's signature, address, and phone numbers
Parents/Guardians are required to provide all insulin supplies, glucagon, snacks, and juice for the student.
